[Bug 4192] - Manual approval of new users under jetspeed

The problem with this and the one below is that we need to select a
template based on a specified locale - the current code assumes that
its the users own locale.

Thus I suggest we add the locale to the locateEmailTemplate method -
since its only being added for this reason.  

Is this ok?

I will do a revised patch and update the email templates bug item.

I assumed that only one of the two will be sent - and thus only need
configuration for one.  The on/off flags indicate how they will be
used.

Do you want the option of separate notifications of new users from
the approval emails?  If so, I presume these would be sent upon the
user being accepted?

I'd prefer not to do this at this stage;  a) cos I don't need it,  b)
the change is big enough already that I want to minimise the initial
patch for it.  We can then refine it.
