Here's a first shot for structure information. Referenced content items are still missing. At least there are objects related to the references instead. Probably there must be some class similar to the org.apache.pdfbox.util.PDFStreamEngine that extracts content items from a page's content stream and creates objects for these containing the MCID if there is one.
