The following scenario is working:
1) Deploy Windows 2012 VM with non-vGPU service offering.
2) Install PV drivers
3) Change service offering to the one with vGPU.

VM comes up fine after that.

For scenario, to install PV drivers after VM with vGPU offering, i am not sure if it a cloudstack bug; from logs, more likely it looks like XenServer issue. I am investigating it and I'll update the ticket once i get more details.
