Attached patch fixes at least the behaviour you are seeing. An expires parameter
is now returned regardless of it's cached state.
@Cocoongods: please review and reject/apply. It's a 5-liner.

I did not see the duplicate content-length header problem you're describing. Can
you specify a bit more how and when you're experiencing this?
