Contents of NOTICE and LICENSE files - official documentation

There does not seem to be any clear definition of what is to be
included in the NOTICE and LICENSE files.

NOTICE
======
The page
http://www.apache.org/legal/src-headers.html#notice
describes how the NOTICE file must begin, and what may be added to it, i.e.
* required third-party notices
* may also include copyright notices

However, there does not seem to be any definite ruling on what should
*not* be put in the notice file. There are some examples of NOTICE
files that contain 3rd party license headers, which is wrong AFAIK, so
the description should say so.

also included some examples of copyright notices.]

LICENSE
=======
It is clear that the LICENSE file must contain the AL2.0; however
there does not seem to be any documentation on what else it can or
should contain.

In particular, there seems to be confusion about how 3rd party
licenses are to be handled.

AFAIK, these can either be added to the end of the LICENSE file, or
stored in a separate file which is referenced from the main LICENSE
file.

But I cannot find any documentation that supports this on the Apache web-site.

Have I missed the documentation?
