sunShine portal question

I am getting started with the portal.  I would like to know is there are any 
examples of having a form as a sunlet, to provide something like a survey 
feature.  Also I would like to have the sunlet form post to a java custom 
java class.  Does anyone have any sample code that is like this?  Any help 
greatly appreciated.
