Error pages are displaying server signature details despite ServerSignature
being set to Off.

I am running Debian Sarge, a fresh install as of about 3 months ago.  I'm using
Apache 2.52.  I've searched all the config's I can find and done recursive greps
to find any that could have been hiding and find no other instance of
ServerSignature, but the error pages still produce a signature.  After some
investigation, it looks as though it's simply a bad setup in the bottom.html
include file.  This extra information only shows up when mod_include is being used.
