XMLSerializer produces malformed document

XML Serializer is also fixing up namespaces during the serialization.. and it 
did not do a good job with default namespace declarations. I've put the fix in 
CVS, you can the new jars on tomorrow at 
http://gump.covalent.net/jars/latest/xml-xerces2/.
Please, test it -- there might be more cases that we overlooked.
