There was a thread on fop-user about this (over a year ago), in which the
last message[1] contained an example of an implementation of this for
version 0.20.5. Might be of some help. Unfortunately, 0.20.X is frozen, so
the changes were never (and never will be) committed.

If you're interested and willing, please have a look at the current
development version (from CVS). If you could implement it on that branch,
I'm sure it will be much appreciated by many users in the future.

Also, the following wiki page[2] describes some of the theory behind having
Bidi support in FOP.
