#### What does this PR do?
Updates to the x509v3 shared CA certificate generation script.
Bugs Fixed:
1) Shared CA x509 SSL/TLS certificate no longer generated with 30 day expiration.
2) X509_CA_* environment variables always used the defaults
3) X509 CA certificate was missing the "basicConstraints=CA:FALSE" directive causing some browsers not to validate
4) Misc fixes to the script and cleaned up syntax/indenting.

#### What is the best way to verify this PR?
1) Remove the existing traffic_ops CA directory:
```
rm -rf $traffic_control_src/infrastructure/cdn-in-a-box/traffic_ops/ca
```
2) Rebuild traffic_ops perl docker container and start CIAB
```
# from cdn-in-a-box
docker-compose -f docker-compose.yml build trafficops-perl trafficops
docker-compose up
```
3) Verify that the SSL certificates contain the correct dates of validity:
```
# FROM cdn-in-a-box directory
$ openssl x509 -in traffic_ops/ca/CIAB-CA.crt -noout -text | grep 'Signature Algo' -A 6
Signature Algorithm: sha256WithRSAEncryption
