Forms on Apache building by PHP

Hi,
 
I try to build the forms using PHP which will be submitted as an email to 
the mail server. We use Apache 1.3.22 + PHP 4.0 + Mysql on Solaris 8.
Could anyone provide some information or documentation about how to
build it?
