Removing deprecated code

The last big issue before declaring 1.0 as 'code complete' is
https://issues.apache.org/jira/browse/SHIRO-155

I just wanted to state a fair warning that I'm starting this effort now.

In practice, this won't take me long at all (30 min? 1 hour max?)
since none of the existing Shiro code depends on Deprecated classes or
methods.  Just a heads up!
