Synopsis: 
Form based authentication gives 404 - File not found error.

Description:
When you access the form directly (most often done by bookmarking it) and not by accessing a protected area first, after submitting the username and password Tomcat returns a 404 error. This because it does not know which page to send after the user has been authenticated. The result is that a null is appended to the end of the url and this does not exist.
