[Bug 122726] Insert picture from file - previous image retained

I opened bug 123922 for the "replace a picture when a picture is selected"
behavior (item 2 of the original post), since the discussion here mostly
concerned drag&drop.

So discussion about item 2 can continue there (and my opinion is that the
behavior changed, but the old one should be restored).
