There's something missing in your report. I'm getting (of course):

[Wed Aug 06 17:21:16 2003] [error] [client 127.0.0.1] (2)No such file or
directory: Could not open password file: /foo

with the configuration:
    AuthType Basic
    AuthName test
    AuthUserfile /foo
    require valid-user
