If mgt server is rhel/centos, need to download from this url, then copy vhd-util into /usr/lib64/cloud/common/scripts/vm/hypervisor/xenserver/
If mgt server is ubuntu, then put vhd-util into /usr/lib/cloud/common/scripts/vm/hypervisor/xenserver/vhd-util

About (1), we aren't talking about frozen guides that are shipped as PDF and are by definition constantly out of date. Our stuff will be updated live on the docs site within minutes of checkin. "An online resource where one could get latest information right away" = our docs site. Yet those docs will undergo some review process. David's point about controlling contributions is perhaps the main point. There is no advantage in having information that is updated quickly but is incorrect.
About (2), I thought that was exactly the approach agreed. We would not document where to get binaries in the guides, only how to go from source.

Don't get the stuff too complicated... We just document that installing the vhd-util is a must, no matter which release/distribution you are using, if you want to use xen hypervisor:

So according to Rohit's comment above about updating the nonoss patch to revert/include vhd-util, the procedure to install this manually is not required if you are using a non-oss build, only if you are building yourself from code or using some Apache CS provided build. Correct?
Also, it is stated you must do this "during management server installation." Could you please say where exactly in the documented procedure this would be done -- at what step? Usually things have to be done in a certain order.

Affirmative, for nonoss build there is no need to install the vhd-util manually. The nonoss build is patched with the same vhd-util as in http://download.cloud.com.s3.amazonaws.com/tools/vhd-util

1. The wiki can't be our official documentation. (we allow anyone to participate there, we would have to restrict that to folks who are committers if we want our wiki to be our official project documentation)
2. The project is only distributing source builds. Wido is distributing his own binaries - which make no use of install.sh - and jenkins.cloudstack.org is producing builds with install.sh that aren't for general public consumption (the builds of the commit that is the current 4.0.0 are long since gone). So if the install documentation doesn't document installing - what are we doing shipping it?

Documenting and releasing guides are definitely nice way to bundle them with a release but this only works for people who will use either the opensource ACS release etc. and works for CCP.
About 2., this makes things very tricky. May be we can document how one can build their own packages (https://cwiki.apache.org/confluence/display/CLOUDSTACK/Building#Building-Packaging) and as that bundles install.sh, they can use install.sh. Also, since ASF won't do any official binary release, may be we should not link either of the jenkins.cloudstack.org or Wido's repositories in the guides?
