SQL Extension Enhancement Files

I had a bit more time to work on this, so I cleaned a few things up and
fixed a couple small problems.

Attached are the latest files for my proposed enhancements to the Xalan SQL
extension. I believe that all of the defaults are set so that pre-existing
stylesheets should not be broken by these changes.
