2) right click TOC - Update Index/Table
TOC disappears, undo disabled.
3) close document without saving
4) reopen document
5) save as odt
6) close document
7) reopen document
8) right click TOC - Update Index/Table

Effect is reproducible with "AOO 4.1.0 RC3 – English UI / German locale
[AOO410m17(Build:9763)) - Rev. 1586584 2014-04-11 08:56:50]" on German WIN7

versions, in a much more simple way: simply open document, rightclick TOC and
update
But anything else is completely unclear
We still need research concerning
(a) Version of appearance
(b) roots: Document does not contain headings, so the question is
why the TOC contains something when document becomes opened.
May be the problem ha nothing to do with TOC, but with import of
Headings or similar

a): Already the same with OOo 1.1.5
b): I get a similar TOC to Screenshot with
11. open sample document
12. 'rightclick TOC -> Update'
> TOC Contents lost as reported
13. 'rightclick TOC -> Edit Index -> check "Create from additional Styles"
-> Styles Browser -> "Default" to level 1 -> [ok] [ok]
> Entries reappear (with wrong formatting what is a different issue)
(c) Due to "b" I think it's an open-import problem, TOC looses info concerning
Additional Styles for TOC entries (this only is a hypothesis)
(d) More or less Reproducible with LibO 3.5.7.2 Build 350m1(Build:2)
on UBUNTU Linux 12.04 x64 (VirtualBox)
