failure in monitor results

hi,
   Even though we have followed the procedure mentioned in 
http://jakarta.apache.org/jmeter/usermanual/build-monitor-test-plan.html    
we are not getting the status of server and graphs specified.

Specifications:

Web server: Tomcat5.0
Path: /manager/status/
Loop count: forever
Thread group: 1
Constant timer: 5000msec 

We are not getting graphs.Even though we are getting response from it is
indicating server status as dead.
can you provide us some suggestions regarding monitor results in jmeter.
