When you upgraded CloudStack to 4.4.0, it must have failed at some other point. By running 421-to-430 sql the issue of failing due to duplicate column is expected. Can you share logs from the management server which started and failed, the real issue would be in that log.

4.2.1 users have been able to upgrade to 4.3.0 in past so probably it's not the 4.2.1 to 4.3.0 upgrade path causing the error for you. Can you also share what ACS 4.2.1 build you're using noredist (nonoss) or oss?
