Currently all (both) QueryIndex impl output a constant 1.0 cost value.

This makes for a difficult choice on the QueryEngine side of things, which currently keeps the first index it sees.

For example a lucene can trump a propertyindex in a uuid resolve query, even though it shouldn't.
