This is confusing and apparently two things. What is an "xobject text stream"? A PDFormXObject? PDFs are not really meant for update and it will likely be a pain. You can access everything low-level by calling getCOSObject().

Re structure tree element there I can't help - have a look at the PDF specification. There are a few answers on stackoverflow, but these were about creating IIRC.
