This attachment contains input file, HTML file containing data in HTML format
and the generated Open Office Output file.
We are trying to generate a open office report having a placeholder in ODT file
which contains dynamic HTML.
The issue is precisely with the Bullet indentation at Level 2 and Level 3.
Please find the attached word file which contains the data in HTML format.
Also find the attachments which contains input data and generated open office
word output respectively.

I don't understand what you means.
See specification for <INPUT> tag:
http://www.w3schools.com/tags/tag_input.asp
value attribute must be a text and not an external document.
