To reproduce, try creating a new document in Futon. Add one or more fields, make sure you actually call them something other than "unnamed...", and then delete them (without saving the document in the interim). Save the document, and the deleted fields will reappear. This doesn't happen if you don't change the field names from "unnamed...".
