SOAP-ENV:Server.BadTargetObjectURI

Linux 7.0, Tomcat 3.2.1, Xerces 1.2.1 and Soap 2.1

I was having trouble with Xerces 1.3.1 with fault code element and resolved
that issue with Xerces 1.2.1; however, now I am getting
SOAP-ENV:Server.BadTargetObjectURI

Fault string: unable to resolve target object: Exchange

This looks like a configuration issues; however, I don't know where to fix
it.

I am trying to run example outlined in Graham Glass article:
