Problem with UsernameToken handling

All,

as Christof pointed out there are some prblems with
UsernameToken handling. I'll check the problems during
the next time.

I also need to update the documentation, plus some code
because the changes in the OASIS WSS Specs.

This problem is only in the Axis handlers, not in the
WSS4J components.
