I was able to reproduce the bug #1 given the instructions from the reporter.
Upon saving the document and opening it up in Microsoft Word, the table’s
second page header was missing and only the first page header was shown (as
described). This appears after formatting the saved file either with ODT format
saved options or without, and is reproducible time after time again.
I went a step further and was able to reproduce this for the saving format of
any Microsoft version (including saving as a doc (Microsoft 95)). It appears
that this is a problem when the file is translated into a .doc that Microsoft
Word is expected to open. 
This is a huge problem if tables are used a lot in any format and then saved
into a doc via the Microsoft Word save option in OpenOffice. Although it may
appear to be a minor issue, over time this can lead to tables missing headers
that can have highly important information in them just due to the page break
being at an odd place.
