Template for Load Testing

Hi,

I am in need for creating a template for doing the Load Testing.
With the Template there is also a need to prepare the GuideLines for
doing the Load Testing.

Both these documents should be CMM Level 5 compliant.
Any guidance in this direction will be helpful.
