Part of #4070.
### Goal
Log database connection-pool initialization in `SqlServer`.
### Problem
Connection-pool init/replacement was silent (the DAO module has no scala-logging dependency).
### Scope
- INFO on pool init/replacement (JDBC URL only, never credentials), via slf4j.
- Non-functional; no behavior change.
