I've never seen SA add headers to any encapsulated email. When it encapsulates
spam, it keeps there whatever headers it found there originally (rather, it
makes no changes to that email), and it places a minimal/necessary set of
headers, plus its own X-Spam headers, into the outer email.

What email client does your customer use to open their email? How is that email
then stored on the system where they try to open it and have problems with the
RTF attachment?

Your Nov 19 attachment here is unusable by any email client I have on my
machine. But if we can identify the software involved, we might be able to find
a way to obtain the email in a form where someone can see where the problem lies.
