fix(ai): gate the break-glass mutation tools at the declared L3 level

rmq.group.reset_offset and rmq.message.redelivery_dlq declared break_glass
and reason in their input schemas but carried riskLevel L2, so the mutation
filter skipped the studio.ai.allow-l3-tools gate, the break-glass approval
and the mandatory reason for two delete-equivalent mutations (offset reset
can skip or replay messages at scale; DLQ redelivery mass-resends). Promote
both tools to L3 to match their declared fields, their destructiveness and
their L3 siblings (rmq.group.delete, rmq.topic.delete, rmq.acl.delete,
rmq.user.create, rmq.user.delete), and pin the invariant with a catalog
consistency test plus a mutation-filter test that drives the real
rmq.group.reset_offset definition through all three L3 gates.
