Ugh. I don't think we should be using heuristics like this. It just doesn't make sense. Either we're using the bbox to extract text, or we're not. Using it as a proxy for the glyph's visual bounds is an poor idea, after all we already have access to perfect bounds if we want to use them!
So, either we use the bbox, or we use the visual bounds, but mixing things like this is really messy from a logical standpoint - semantically, it's not _meaningful_ to choose to do this.
