   ## P7 feat(nodejs): consumer-side rate limiting via semaphore backpressure
   
   - `src/util/Semaphore.ts` applied to `ProcessQueue`, bounding in-flight consumption by
     `consumeConcurrentlyMax` (default 32); the receive loop pauses while all permits are
     taken, providing per-queue consumption backpressure.
   - `PushConsumerOptions#consumeConcurrentlyMax`; broker-side value parsed from settings
     when present.
   
   **Tests:** `nodejs/test/util/Semaphore.test.ts` + `nodejs/test/consumer/ProcessQueue.test.ts` (10 cases, offline, all pass)
