This PR takes care of two things:
1 - As previusly agreed on the mailing list, this PR adds a new API `TSHttpTxnServerSsnTransactionCount` to retrieve the number of transactions between TS proxy and the origin server from a single session. This new api uses the `sstc` field to indicate connection reuse.
2 - A new `header_rewrite` condition named: `SSN-TXN-COUNT` that uses this new API.
