KahaDB index keeps growing over time which occupies broker disk space.

The index does not shrink when in-use journal files are cleared after messages from the broker are consumed.

Further, on unclean shutdown of the broker the known free pages are lost and no longer tracked. This causes new pages to be created which takes up further more disk space.
