I'm using php-fpm as my fastcgi process manager.

Version 5.4.3 with the following patch applied:

https://bugs.php.net/bug.php?id=62172

It gets php fpm to strip out the proxy:balancer stuff from the SCRIPT_FILENAME
for php, otherwise I'd be getting 404s on all the pages.
