Cover association blocks in PredicateGeneratorSpec

The join handling of association blocks changed in PredicateGenerator but
was only covered end to end. These cases check the joins it builds: the
join type of a nested block, an inner join when none is recorded, the
join type of an alias definition, reusing a join of the same context,
and not reusing a join of an outer block or an outer query of the same
name. The nested join type, the outer block and the subquery cases fail
without the fix.
