Hello, developers!

To my great grief can not render subj.

Failures:

* where are the 
top and bottom rulers I have asked for?
* why are not the PNG images scaled to fit? (docbook itself 
uses "auto" for all, but I have manually changed for scale-to-fit for the content-width - did not 
help)
* where is the numbering for odd pages?
* why are the pages in wrong order?

(these are 
the parameters I pass to docbook-xsl 1.60.1
  paper.type "'A4'"
  header.rule 1
  footer.rule 
1
  double.sided 1
)
