I have a quite big document, +120 pages, in which the titles of chapter are
formatted with "Intestazione1" through "Intestazione4" (in italian version".
When I try to insert an index, with the menu:
Inserisci | Indici e Tabelle | Indici e Tabelle ...
I see for a couple of second the window of the index specification, and then
the program crashes.
The Windows event viewer show me this error

Can you provide a link to the document in question? It'll probably be too big
to upload here.

CRITICAL CONDITIONS (assumption based on original report)
-------------------
- Big document - 120+ pages
- Insert an index
- Titles of chapter are formatted with "Intestazione1" through "Instestazione4"
(Italian version)
- "Interstazione1" means Header1
- Version 4.1.2
PREPARATION
-----------
- Create document with 120+ pages
- Find tool for creating the content
- www.lipsum.com
- Learn how to insert an index
- Download versions from 4.1.1 to 4.1.3
TEST NOTES
----------
CONFIGURATION FOR TEST 1: Windows 7 Enterprise SP1, Apache OpenOffice 4.1.3 -
English as language
TEST 1:
- Created 135 page document (test_document_126880.odt) by generating text from
(www.lipsum.com). I included to each chunk of 15 pages of text one Heading, one
Heading 1, one Heading 2 and one Heading 3. There were in total 36 headings,
but only 27 were shown in index as Heading didn't appear on index (only Heading
1, Heading 2 and Heading 3)
- Insert index via Insert - Indexes and Tables (by using default settings)
--> Index table created in few seconds without problems being noticed
CONFIGURATION FOR TEST 2: Windows 7 Enterprise SP1, Apache OpenOffice 4.1.3 -
Italian as language
TEST 2:
- Using the same document as in TEST 1
- Insert index via Insert - Indexes and Tables (by using default settings)
--> Index table created in few seconds without problems being noticed
CONFIGURATION FOR TEST 3: Windows 7 Enterprise SP1, Apache OpenOffice 4.1.2 -
Italian as language
TEST 3:
- Using the same document as in TEST 1
- Insert index via Insert - Indexes and Tables (by using default settings)
--> Index table created in few seconds without problems being noticed
CONFIGURATION FOR TEST 4: Windows 7 Enterprise SP1, Apache OpenOffice 4.1.2 -
Italian as language
TEST 4:
- I modified the document by increasing the amount of headings to 20 in each
15 pages of text - in other words 180 headings for new index (compared to 27 in
first document) - the length of the document
(test_document_126880_version2.odt) grew to 140 pages
- Insert index via Insert - Indexes and Tables (by using default settings)
--> Index table created in few seconds without problems being noticed
CONFIGURATION FOR TEST 5: Windows 7 Enterprise SP1, Apache OpenOffice 4.1.2 -
Italian as language
TEST 5:
- I modified the document (on TEST 4) by increasing the amount of headings to
63 in each 15 pages of text - in other words 567 headings for new index
(compared to 27 in first document or 180 in second document) - the length of
the document (test_document_126880_version3.odt) grew to 158 pages
- Insert index via Insert - Indexes and Tables (by using default settings)
--> Index table created in few seconds without problems being noticed
CONFIGURATION FOR TEST 6: Windows 7 Enterprise SP1, Apache OpenOffice 4.1.1 -
Italian as language
TEST 6:
- Using the same document as in TEST 5
- Insert index via Insert - Indexes and Tables (by using default settings)
