if you add this document to the db:

and than try to update it

Resolution: Duplicate
Questions like this are a better fit for the mailing list: http://couchdb.apache.org/community/lists.html
I think the answer is that you need to provide the _rev as part of the JSON body. This isn't because you are wrong, it's because CouchDB still has some code that needs to be written. I think this is addressed in COUCHDB-642
