- updates cql version to 3.0
- adds column names and types to the cfdef
- uses those column names to build the statement (prepared statement previsouly used ?=? pairs for columns now uses "C.." = ? pairs)

I think it's to early to move stress to cql3 entirely so, first of all, it would be great to make it optional (e.g. -L3 (--enable-cql3)), and second, there is no notion of SELECT FIRST in CQL3 so you would have to work on CqlReader make it work.

What I couldn't figure out was how to use UUID column names (required when the TimeUUID comparator is set with the U option), that option currently throws an UnsupportedOperationException.
