how to specify footer and headers for new pdf documents?

Does anyone know how to specify the contents of footers and headers on all
pages when creating a new pdf document with pdfbox?  Also, does pdfbox
support including images in the header or footer section?

Hi,

There is no such thing as a "footer" or the "header" of a PDF. Stuff can 
be drawn everywhere.
