Section 18 : Where is , description ?

Hi,

We have <foreign-key> described in Section 18, yet <unique> and <index> are 
not to be found (certainly in the 09-09-2005 variant of the spec, the most 
recent there is). Also in the <foreign-key> section there is no definition of 
the "table" attribute.

Do we have some definition of, for example, where the attribute "table" of 
<index>, <unique> or <foreign-key> will be used ? I'm looking for examples 
too ;-)
