The form definition file \cocoon-2.1
\build\webapp\samples\ojb\woody\employee.xml hasn't been updated according to 
the new markup described in http://wiki.cocoondev.org/Wiki.jsp?
page=WoodySyntaxUpdateForWidgetsElement.

To resolve this issue add a <wd:widgets> to the document element to nest all 
<wd:field> elements.
