@gatorsmile, I've addressed your review comments other than case sensitivity. When you have time, please reply with what you think the case sensitivity requirements should be for a catalog. Specifically:
* Should a catalog be required to support case insensitive table identifiers?
* Should a catalog be required to support case insensitive column updates and column deletes?
