1) Remove unnecessary exceptions (e.g., various subclasses of CacheException, DMLUnsupportedOperationException, AppException)
2) Fuse unnecessary deep exception hierarchies (e.g., DMLParseException)
3) Reinvestigation uncaught vs caught exceptions
