pdf 2.2.2 docs

There were some questions about why a 2.2.2 version of the apache
httpd docs was not available as a pdf file.  I finally found time to
generate one, which is now at
http://apache.org/dist/httpd/docs/
and will be available from the mirrors shortly at:
http://www.apache.org/dyn/closer.cgi/httpd/docs/
