[C2][Actions] shjould actions be run if we're using instance from cache?

How do we currently plan this: if we find a valid cache for a request, 
should the actions be run anyway?

No matter what the answer is, I beleive it might be usefull to have
the actions be settable up for both types of behaviour:

<map:act run="alway">
...
</map>

<map:act run="ondemand">
...
</map>
