Reporter sent a test kit to me (I asked him to attach it here), and if I
understand everything correctly problem is  Reproducible with "AOO 4.0.1   –
German UI / German locale  [Rev. 1524958 2013-09-20 11:40:29]" on  German WIN7
Home Premium (64bit)", “historic”  4.0  User Profile used for all  predecessor
versions:

1. From AOO Start Center open his "overzicht IK tem dec 2012.docx"
2. Open a compy of that document with MS WORD viewer and compare:
   In MS WORD viewer there is some kind of flow chart 
   Bug: In AOO document looks empty

Some modifications here due to my questions:

(a) see new summary!
   This is a Writer - open-import bug
(b) I hope reporter will attach testkit, soon 
(c) see above!
(d) screenshots not required
(e) Still missing. Philppe Duhamel, please contribute info how you created
    "overzicht IK tem dec 2012.docx" (especially: with what software?)
(f) no more info required except (f9)
