Are the toolbars
(a) lost / missing
or
(b) hidden behind an other Toolbar
I already observed (b) from time to time with 4.1.0 until now not reproducible
reliably), but because you observations are 4.2 only that seems to be something
different.

I can confirm the described defect on Windows 7 with my local build of a recent
trunk version.
I could not reproduce the described defect with AOO 4.0.1 or with AOO 4.1.0
RC3.
Taking over this issue because I assume that the changes made for issue 124355
caused this defect.
